Survey of Science, Math, and Engineering Educators with Disabilities

You are being asked to be a volunteer in a research study. To participate, you must read and agree to the following before you may proceed with the survey.

Researchers from Georgia Tech. are conducting a survey to learn about the employment experiences and workplace accommodations used by science, technology, engineering and math (STEM) educators who have a disability. The survey asks about the functional limitations of the educator, employment history, and the types of workplace accommodations used. It also includes typical demographic questions.

We are recruiting up to 500 K-12 and postsecondary STEM educators who have a disability to participate in this survey. The survey takes about 20-30 minutes to complete. While there is no direct benefit to you for participating in this study, the goal of this research is to provide information to help people with disabilities successfully become teachers and to help educators who acquire a disability to be able to continue in their career.

The risks involved with participating in this study are no greater than those you would normally encounter while accessing the Internet on a personal computer at home or work. This survey does not ask for your name or any type of contact information (such as email address). You should be aware, however, that the survey is not being run from a "secure" https server of the kind typically used to handle credit card transactions, so there is a small possibility that responses could be viewed by unauthorized third parties (e.g., computer hackers).

To make sure that this research is being carried out in the proper way, the Georgia Institute of Technology Institutional Review Board may review study records. The Office of Human Research Protection may also look at study records. The sponsor of this study, the National Science Foundation, also has the right to review study records. Again, your privacy will be protected to the extent allowed by law.

You will not be paid for your participation, nor are there any financial costs to you to participate. Your participation in this study is voluntary. You do not have to be in this study if you don't want to be. You have the right to change your mind and leave the study at any time without giving any reason, and without penalty.
